Official Submission Form

Deadline: Feb. 1st, 2008 , at 5 p.m.
English/Literature Department (LA 114) at Utah Valley State College

 Requirements:

• Current UVSC student enrollment

• Completed submission form, including the back

• Printed manuscripts (do not show author/artist name on manuscripts)

• Electronic copy of manuscripts on a CD in .txt, .rtf, or .doc format (visual art must be in .pdf or .jpg format)

• You must include a self-addressed stamped envelope (SASE) for notification purposes.

Recommendations:

• Do not use a pseudonym

• Do not use floppy disks

• Submit art in the largest file size possible to avoid pixelation

• Submit manuscripts, artwork, and CDs in an envelope (do not use staples or binder clips)

Checklist:

• Include this completed submission form in an envelope with your submissions

• Include a CD with electronic copies of your submissions

• Include an SASE (notice will be sent only via SASE)

• Make sure submissions have a clearly printed title but no author/artist names

• Do not submit original art or manuscripts (Touchstones is not responsible for lost or damaged pieces)

• Double check the contact information
